Insurance
If you have insurance, we will file your claim with your insurance company. We ask that benefits be assigned to our office. We consider insurance to be Medicare, Blue Cross Blue Shield, and other insurance plans. Payment of any co-pay amounts will be required at the time services are rendered. If you receive hospital care, we request payment of the co-pay amount within 30 days after your claim is filed. If you have assigned benefits to our office and you have a supplemental policy, we will file your supplemental form.
Supplemental Insurance
If you previously assigned benefits to our office, you will not have to pay the co-pay amount and we will file the claim for supplemental insurance. If your supplemental policy does not pay within 30 days after our filing your claim, you will be responsible for paying the balance due.
Overpayments
Any overpayments made by a patient will be reimbursed within a reasonable amount of time. Any overpayment made by an insurance company will be reimbursed to the insurance company unless due to the patient.
