We realize that health insurance plans and reimbursement policies are frequently frustrating and confusing. If you have any questions or concerns, we encourage you to speak with our staff.

 

How may I pay?  We accept payment by cash, check, VISA, Mastercard and Discover.

 

Does Mountain View Clinic/Central Wyoming Neurosurgery participate with my insurance plan/TPA plan? Mountain View Clinic/Central Wyoming Neurosurgery (MVC/CWN) participates with Medicare, Title XIX, First Choice of Midwest, Greatwest/Cigna, Sinclair, Blue Cross Blue Shield of Wyoming, United Healthcare, Health Info Net, and WISE Provider Network. We also accept Wyoming Worker’s Compensation patients.

 

What is my financial responsibility for office visits and services?

 

If you have…..

 

Commercial/Indemnity Insurance you are responsible for payment of your deductible and co-pay at the time of the visit. You will also be responsible for any services not covered by your plan after claim is processed by your insurance carrier.  MVC/CWN will file the insurance claim on your behalf.

 

PPO Plans you are responsible for payment of your deductible and co-pay at the time of the visit.  You will also be responsible for any services not covered by your plan after claim is processed by your insurance carrier. MVC/CWN will file the insurance claim on your behalf.

 

Out of Network Managed Care Contract you are responsible for payment of your deductible and co-pay at the time of the visit.  You will also be responsible for any services not covered by your plan after claim is processed by your insurance carrier.  You will also be responsible to call your Insurance Company to determine network benefits, co pay, deductible and non-covered services.  MVC/CWN will file the Insurance claim on your behalf.

 

Medicare you are responsible for payment of deductible and co-pay after Medicare has processed your claim.  MVC/CWN will file the Medicare claim on your behalf, as well as any claim to your secondary insurance.

 

Wyoming Worker’s Compensation you are responsible to provide MVC/CWN with the date of injury, claim number, the adjuster’s name and phone number;  no payment is necessary at the time of the visit. MVC/CWN will file the claim on your behalf.

 

Title XIX/Medicaid you are responsible for payment of co-pay at the time of visit. MVC/CWN will file the claim on your behalf.

 

Uninsured/Self Pay you are responsible for payment at the time of the visit. If surgery is required a 25% down payment is required before the procedure. MVC/CWN will assist you setting up arrangements for payment of the balance of your account. 

 

Third Party Liability and Accident Victim you are responsible for payment at the time of the visit. MVC/CWN will file the claim on your behalf according to the rules stated by your primary insurance carrier.

 

Personal Injury and Motor Vehicle Accident you are responsible for payment at the time of the visit.  Upon request MVC/CWN will provide your attorney all necessary paperwork related to your case. (copies and reports have an additional charge).

 

Litigation: We DO NOT accept responsibility for claims in litigation. Payment is expected directly from the patient at the time of service.
